About The Position

Our inpatient rehabilitation hospital is committed to providing exceptional and compassionate care to best address the medical, physical, emotional, and vocational challenges for individuals with brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas.

Requirements

  • Current state licensure or eligibility for state licensure as a Physical Therapist.
  • Certified BLS.
  • Per Diem Requirement: Two (8 HR) shifts per month, at least one of those a weekend and one high need day (week day, weekend, or holiday).

Responsibilities

  • Evaluate all assigned patients and develop, implement and modify treatment plans in accordance with the patient’s needs and physician direction.
  • Delegate and direct all discipline-specific therapy services for assigned patients.
  • Design functional treatment programs allowing for an effective return to community activities.
  • Serve as an educator of patients, families, students, co-workers, and self.
  • Perform other professional duties associated with the care of the patient, development of discipline-specific therapy services, and design of program delivery.
  • Provide ongoing evaluation of patient’s status and goals, modifying treatment programs for timely progression.
  • Develop comprehensive goals for patient treatment to maximize patient outcomes and return to community activities.
  • Coordinate treatment programs in conjunction with other clinical/therapeutic services and available community resources.
  • Attend appropriate clinical meetings to facilitate coordination.
  • Provide patient education, including the development of comprehensive and progressive home programs with instruction to the patient, family, and other caregivers.
  • Prepare for patient discharge, including ordering necessary equipment and recommendations for therapeutic services to ensure the continuum of care.
  • Evaluate patients in actual or simulated home, school, work, or community environments as necessary to facilitate a return to community activities.
  • Identify and manage various levels of care necessary to ensure care plan delivery.
  • Delegate and supervise the utilization of PTAs and support personnel to maximize patient outcomes and ensure effective utilization of services.
