About The Position

The position is for a Licensed Physical Therapist at Advocate Health, specifically in the Outpatient Rehabilitation department. The role involves interpreting physician referrals, conducting various evaluations, establishing treatment plans, and monitoring patient outcomes. The therapist will also be responsible for patient discharge planning, documentation, and maintaining productivity standards. The clinic operates Monday to Thursday from 6:30 am to 7 pm and Friday from 6:30 am to 3 pm, with weekends closed. A sign-on bonus of $5000 is offered, payable in two installments. The pay range for this position is $40.30 to $60.45 per hour.

Requirements

  • Doctorate degree in Physical Therapy from an accredited school.
  • Licensed Physical Therapist in the state of practice.
  • Basic Life Support (BLS) certification within 90 days of hire.
  • No prior work experience required.
  • Intermediate computer skills.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to demonstrate knowledge of patient care principles across the lifespan.

Nice To Haves

  • Herman & Wallace Pelvic Rehabilitation Practitioner Certification (PRPC) or equivalent.

Responsibilities

  • Interprets physician referrals and completes evaluations.
  • Establishes and implements individual treatment plans based on evaluations.
  • Monitors and evaluates treatment outcomes, adjusting plans as necessary.
  • Identifies and addresses factors affecting patient comfort.
  • Plans and completes patient discharge, including home programs and referrals.
  • Communicates with stakeholders to ensure optimal patient care.
  • Completes required documentation following department policies.
  • Records patient care and progress.
  • Maintains productivity standards and adheres to billing policies.
  • Cleans and organizes the treatment area after patient sessions.
  • Demonstrates skill in the use of modalities and exercise equipment.
  • Participates in safety initiatives and follows infection control measures.
  • Engages in performance improvement activities.
  • Manages assigned caseload and informs leadership of coverage needs.
  • Pursues professional growth through continuing education.
  • Orients new staff and participates in the evaluation of co-workers.
  • May supervise physical therapy assistants and students.
