About The Position
We're a dedicated home health team in Honolulu focused on wellness, recovery, and fall prevention for older adults across Oahu. Our mission is simple - to help clients regain confidence, mobility, and independence through compassionate, skilled therapy. We're looking for a licensed Physical Therapist or Occupational Therapist who enjoys autonomy, flexibility, and the satisfaction of seeing patients thrive in their own homes and communities.
Requirements
- Current Hawaii PT or OT license
- 1+ year of experience in clinical, rehab, or home health settings
- Excellent communication and documentation skills
- Confident with patient education and community outreach
- Valid driver's license and dependable transportation
Responsibilities
- Provide high-quality PT or OT services in clients' homes and senior living settings
- Complete initial evaluations and personalized treatment plans
- Perform ongoing reassessments and adjust care plans as needed
- Document progress and communicate effectively with the care team
- Participate in local wellness programs and represent our agency at community events
