Interim HealthCare of Montgomery County is seeking a compassionate and skilled Physical Therapist (PT) to join our growing hospice team . This role focuses on improving quality of life, comfort, and functional mobility for patients receiving end-of-life care in the home setting. Requirements

  • Graduate of an accredited Physical Therapy program
  • Active Tennessee PT license (or license eligible)
  • Strong clinical judgment with a patient-centered, compassionate approach
  • Excellent communication and caregiver education skills
  • Ability to work independently in a home-based setting
  • Ability to assist patients with mobility and transfers
  • Prolonged standing, walking, and occasional lifting as required

Nice To Haves

  • Hospice or home health experience preferred , but not required

Responsibilities

  • Perform initial and ongoing physical therapy assessments for hospice patients
  • Develop and implement individualized care plans focused on comfort, safety, and mobility
  • Provide therapeutic interventions including positioning, transfer training, and pain-relief techniques
  • Educate patients, families, and caregivers on safe mobility and home care strategies
  • Collaborate with the Interdisciplinary Group (IDG) including nurses, social workers, and physicians
  • Maintain accurate and timely clinical documentation in compliance with regulatory standards
  • Participate in care planning and team meetings as needed
