We are currently looking for a compassionate, organized, and team-oriented Home Health Physical Therapist (PT) - Level 3 to serve patients throughout the San Mateo Peninsula. In this essential role, you will collaborate closely with our interdisciplinary team, including nurses, therapists, and administrative staff, to provide individualized care and ensure high-quality patient outcomes. Requirements
- Current California PT License (required)
- Valid California Driver’s License
- Experience: Minimum 2 years of Home Health experience required. Must be proficient in OASIS and case management.
- Experience: 4+ years of Home Health experience. Must be an expert in complex case management. Clinical specialization is a plus.
- Coverage Area: Regional Floater. Must be willing to cover the wider service area based on highest priority patient needs.
- Schedule: High Availability. Willingness to work weekends or urgent coverage shifts as needed.
- PTA : Substantial PTA supervision
- Ability to travel within San Francisco and surrounding areas
- Caring, compassionate personality with strong communication skills

Responsibilities
- Provide physical therapy services within the scope of practice, following individualized plans of care
- Deliver treatment, evaluate patient response, and report changes to the Physician, Case Manager, and/or Director of Nursing
- Maintain appropriate clinical records and submit documentation on company devices per organizational policy
- Follow all company policies and procedures
- Perform other duties as assigned
