About The Position

We are seeking a licensed Physical Therapist to provide in-home therapy services across the South Coastal Territory. The PT will deliver care in accordance with a physician’s plan, helping patients regain mobility and independence.

Requirements

• Active California Physical Therapy License
• Minimum 2 years of physical therapy experience (home health preferred)
• Strong communication and organizational skills
• Current CPR certification
• Valid driver’s license and insured, reliable vehicle

Responsibilities

• Evaluate patients’ functional status and develop care plans
• Assess home environments and recommend modifications
• Provide direct physical therapy treatments
• Educate patients and caregivers on therapeutic techniques and equipment
• Monitor progress and adjust treatment plans
• Collaborate with physicians and interdisciplinary teams
• Document clinical notes and progress reports
• Participate in discharge planning
• Supervise Physical Therapy Assistants per policy and regulations
