The PT will deliver care in accordance with a physician’s plan, helping patients regain mobility and independence.
Requirements
- Active California Physical Therapy License
- Minimum 2 years of physical therapy experience (home health preferred)
- Strong communication and organizational skills
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle

Responsibilities
- Evaluate patients’ functional status and develop care plans
- Assess home environments and recommend modifications
- Provide direct physical therapy treatments
- Educate patients and caregivers on therapeutic techniques and equipment
- Monitor progress and adjust treatment plans
- Collaborate with physicians and interdisciplinary teams
- Document clinical notes and progress reports
- Participate in discharge planning
- Supervise Physical Therapy Assistants per policy and regulations
