About The Position

Empath Home Health is growing! We're expanding our home health services in west Pasco County and looking for a passionate PRN Home Health PT Physical Therapist to join our team. This is an exciting opportunity to be part of a market expansion, serving patients in their homes with the flexibility to set your own schedule and grow with us as we continue to build our presence in the community. In this PRN role, you'll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence. Locations: home health visits in west Pasco County (New Port Richey, Hudson, Holiday, Trinity, Odessa, and surrounding areas). If desired, visits may also be available in east Pasco, Pinellas, and/or Hillsborough counties! Schedule: PRN role offering 5-10+ visits per week. Required weekend/holiday on-call rotations. Requirements

  • Active Florida PT license
  • Minimum of 1 year of direct patient care experience; prior home health PT experience preferred
  • Strong assessment, critical thinking, and patient education skills
  • Willingness to participate in weekend and holiday on-call rotations

Responsibilities

  • Perform comprehensive patient assessments, including OASIS assessments at required intervals
  • Develop, initiate, and revise individualized plans of care as patient needs evolve
  • Deliver skilled physical therapy services and treatments as outlined in the plan of care
  • Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
  • Collaborate with the interdisciplinary team to ensure seamless, holistic care
