About The Position

The Physical Therapist (PT) - Home Health Case Manager position is located in the Southeast City with a focus on the communities of 60620, 60619, 60617, 60643, and 60628. This full-time role requires a commitment of 40 hours per week, primarily during the day. The position offers the opportunity for a sign-on bonus of up to $10,000, contingent upon continued employment in the same role at the time of payout. Physical Therapists at Advocate at Home operate independently within a supportive clinical team, assessing and addressing the mobility, safety, communication, and equipment needs of patients. They establish strong relationships with patients and families while collaborating with physicians and other healthcare providers as necessary.

Requirements

  • Licensed Physical Therapist in the State of Illinois (or eligible for licensure).
  • Completed 1 year of Physical Therapy clinical experience or strong clinical affiliation experience.
  • Proficient use of home-based therapy equipment and assistive devices.
  • Basic knowledge of computer use (e.g., Microsoft Word, email).
  • Ability to learn to use computer-based scheduling and documentation systems.
  • Strong verbal and written communication skills with good documentation abilities.
  • Good interpersonal skills, time management, and self-organization skills.

Responsibilities

  • Travel extensively within geographical boundaries in all weather conditions.
  • Work rotating weekends and holidays as required.
  • Adhere to scheduled days and hours of work based on patient/client commitments and workload.
  • Attend mandatory informational, educational, and branch meetings.
  • Perform a full range of body motions including bending, sitting, standing, climbing stairs while carrying objects, pulling, pushing, reaching, carrying, and lifting (minimum of 25 pounds).
  • Assist and maintain patient safety with ambulation on flat surfaces and stairs, and transfer patients using standard pivot transfers or with the use of transfer boards or Hoyer lifts.
  • Assess, plan, implement, and evaluate physical therapy treatment programs for home care patients.
  • Facilitate the progression of patients towards optimal levels of self-care.
