About The Position

H2 Health Prime Living is seeking a Physical Therapist to provide 1:1 patient care in luxury Senior Living communities in Lehigh Valley, PA. The position involves 35-40 visits per week with a flexible schedule. The ideal candidate will have experience evaluating and treating a primarily geriatric population, including patients with Alzheimer's and Dementia related conditions. Requirements

• Graduate of an accredited Physical Therapist program.
• Active or eligible state license to practice.
• Prior experience is preferred, but new graduates are welcome.

Responsibilities

• Provide 1:1 physical therapy care to residents in personal care and memory care communities.
• Evaluate and treat a primarily geriatric population.
• Collaborate with an interdisciplinary team to deliver the best therapy opportunities.
• Advocate for patients and support their needs within the community.
