The position involves evaluating patients within twenty-four (24) hours or 1 business day of physician referral. If unable to complete the evaluation in this timeframe, the medical record must clearly document the cause for the delay. The role includes developing effective treatment plans and obtaining approval for services from the referring physician, as well as treating patients according to the physician's treatment plan. Additionally, the position requires assisting the nursing department with training Restorative Aides and supervising Physical Therapy Assistants in direct patient care and related activities, in compliance with state practice acts. Communication with supervisors and other health team members regarding patient progress, problems, and plans is essential. Participation in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences is also required, along with involvement in in-service training programs for other staff in the facility. The role includes recording evaluations, daily treatment notes, weekly progress notes, and recertification documentation in accordance with PT Board State Practice Act and governmental and third-party payer requirements. Treatment changes must be recorded per policy and procedures, and the position involves instructing patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services. Discharge planning participation and recommending necessary durable medical equipment for patients to facilitate independence in mobility are also key responsibilities. Reporting any problems with department equipment to ensure it is maintained in good working order is required.
