Position Summary The Physical Therapist- PRN provides high-quality, person-centered therapy services to residents and patients within Fleet Landing. This role evaluates, plans, and implements individualized treatment programs that promote independence, mobility, and optimal physical function — supporting residents in achieving their highest level of well-being.

Requirements

  • Education: Bachelor’s or Master’s Degree in Physical Therapy
  • Licensure: Current Florida State Licensure as a Physical Therapist
  • Certification: Current CPR certification
  • Experience: Clinical experience working with older adults preferred
  • Other Requirements: Valid driver’s license and satisfactory driving record
  • Proof of auto insurance
  • Ability to read, write, and communicate effectively in English
  • Frequent lifting and transferring of residents (over 100 lbs with assistance)
  • Occasional lifting of supplies (up to 50 lbs) and pushing/pulling residents in wheelchairs (up to 300 lbs)
  • Standing and walking for extended periods; ability to climb at least two flights of stairs

Responsibilities

  • Provide physical therapy services according to physician orders and accepted professional standards.
  • Perform comprehensive evaluations and develop individualized care plans in collaboration with the care team.
  • Deliver therapeutic treatments addressing mobility, balance, strength, and functional independence.
  • Conduct screenings for new residents and reassess long-term residents as needed.
  • Maintain accurate and timely documentation, including evaluations, progress notes, and discharge summaries.
  • Participate in care planning, interdisciplinary meetings, family consultations, and in-service training.
  • Ensure compliance with all safety, infection control, and resident rights policies.
  • Maintain a warm, respectful, and professional atmosphere for residents and families.
  • Perform other duties as assigned.
