Requirements

• Licensed Physical Therapist in the State of Nevada
• Degree from a CAPTE-accredited program; passed state-approved exam
• Minimum 2 years’ experience (home health preferred)
• Current CPR certification
• Valid driver’s license, insurance, and reliable transportation

Responsibilities

• Develop and update individualized care plans
• Provide physical therapy services per physician’s plan of care
• Assess functional status, mobility, and home environment
• Educate patients and families on therapy techniques and equipment
• Maintain accurate documentation and communicate progress to physicians
