Requirements

A degree in Physical Therapy from an accredited school/program.
A current, unrestricted Maryland license as a Physical Therapist
Demonstrated proficiency in clinical assessments, documentation, and compliance with policies and procedures.

Nice To Haves

Experience in Home Health Therapy and completion of OASIS documentation is preferred.

Responsibilities

Conduct comprehensive assessments of patients' physical abilities, mobility, and rehabilitation needs in their home environment.
Develop and implement personalized treatment plans tailored to each patient's specific goals and conditions.
Provide hands-on therapy techniques, including exercises, manual therapy, and modalities to improve patients' strength, balance, and coordination.
Educate patients and their families on exercises, safety measures, and home modifications to facilitate recovery.
Collaborate with other healthcare professionals to ensure a holistic approach to patient care.
Maintain accurate and timely documentation of patient progress and treatment outcomes.
Stay updated on best practices and advancements in physical therapy and home health care.
