About The Position

The Physical Therapist - Per Diem role at Maui Lani Elua Medical Offices involves providing physical therapy evaluation, management, and treatment services to patients referred by physicians. Requirements

  • Bachelors degree in physical therapy from an accredited college or university.
  • Physical Therapist License (Hawaii) required at hire.
  • National Provider Identifier required at hire.
  • Basic Life Support certification required at hire from American Heart Association.
  • Ability to physically assist patients in performing functional tasks and therapeutic exercises.
  • Demonstrated knowledge of interpersonal relations, oral and written communication, and teamwork.

Nice To Haves

  • Minimum one (1) year of related experience in acute care, skilled care facility, sub-acute care, or outpatient settings.
  • Doctoral degree in physical therapy.

Responsibilities

  • Greets patient, establishes rapport, and orients patient to physical therapy.
  • Conducts initial evaluation, including interpreting referral information and documenting patient concerns.
  • Develops treatment plan with guidance from clinical guidelines, including problem identification and measurable goals.
  • Discusses treatment plan with patient and/or family and communicates it with referring physician and support staff.
  • Manages patient cases through therapy, delegating and instructing clinical support staff as needed.
  • Provides or supervises physical therapy interventions based on examination results and treatment plans.
  • Reviews treatment documentation and revises treatment plans as necessary, communicating changes to customers and support staff.
  • Establishes discharge plans and documents discharge summaries, communicating them appropriately.
  • Participates in program development initiatives and demonstrates innovation in addressing customer needs.
  • Exercises positive listening and communication skills, contributing to team dynamics.
  • Participates in problem-solving and quality improvement activities, demonstrating professional growth.
