POSITION SUMMARY: Provides consultation, evaluation, and physical therapy services to patients across the age continuum. Establishes a rehabilitative plan of care with realistic goals following a functional evaluation for all patients. Provides education to patients, family members, students, staff, and to the community at large. Coordinates services with other medical disciplines, making recommendations as appropriate to facilitate a complete rehabilitative process. Participates in Performance Improvement and Risk Management activities.

Requirements

  • Graduate of a physical therapy program approved by the American Physical Therapy Association.
  • Currently licensed in the State of New York.
  • Participates in orientation and continuing education and updates and maintains knowledge and skills related to specific areas of expertise.
  • Demonstrates age specific competency annually on age populations served as identified in scope of care and services.
  • Strong oral and written communication skills.
  • Strong interpersonal skills.
  • Strong time management skills.
  • BLS for Health Care Providers is required.
