Physical Therapist Pediatric Home Health PRN Service Area: Greater Nashville Area Schedule: Monday–Friday PRN Hours Make a Difference Where It Matters Most Vanderbilt Health is seeking a compassionate and skilled Physical Therapist to join our Pediatric Home Health team on a PRN basis. This is a unique opportunity to provide one-on-one, meaningful care to children in the comfort of their homes across the Greater Nashville area—all while enjoying a flexible weekday schedule.

  • Deliver skilled physical therapy services to pediatric patients in their homes.
  • Develop and implement individualized care plans in collaboration with families and care teams.
  • Document patient progress and outcomes using EMR systems.
  • Participate in interdisciplinary case reviews and care coordination.
  • Analyzes comprehensive information from formal and informal assessment that is pertinent to the patient's care and management to determine a rehabilitation treatment diagnosis.
  • Collaborates with stakeholders in establishing the plan of care, coordinates and implements care delivery while anticipating patient care needs across the rehabilitation continuum.
  • Applies appropriate, evidence-based interventions to ensure effective outcomes and re-evaluates performance to assess patient's progress towards goals.
  • Employs strategies to promote successful return to home, community, and school/vocational environments.
  • Licensed Physical Therapist in the state of Tennessee.
  • Experience in pediatrics, especially with medically fragile infants and children.
  • Acute care or home health experience preferred.
  • Proficient in the use of Electronic Medical Records (EMR) systems for documentation and care coordination.
  • Strong communication and organizational skills.
  • Passion for working with children and families in a home-based setting.
  • CPR certification and valid driver’s license required.
