Vanderbilt University Medical Center (VUMC) is seeking a compassionate and skilled Physical Therapist to join our Pediatric Home Health team on a PRN basis. This role involves providing physical therapy services to pediatric patients who have impairments, functional limitations, disabilities, or changes in physical function and health status resulting from injury, disease, or other causes. The position requires active collaboration with the healthcare and educational team to meet patient care, outcomes, and system improvement objectives. The Physical Therapist will deliver one-on-one, meaningful care to children in their homes across the Greater Nashville area, enjoying a flexible weekday schedule without weekends or holidays required.

  • Deliver skilled physical therapy services to pediatric patients in their homes.
  • Develop and implement individualized care plans in collaboration with families and care teams.
  • Document patient progress and outcomes using EMR systems.
  • Participate in interdisciplinary case reviews and care coordination.
  • Analyze comprehensive information from assessments to determine rehabilitation treatment diagnosis.
  • Collaborate with stakeholders in establishing the plan of care and coordinate care delivery.
  • Apply evidence-based interventions to ensure effective outcomes and re-evaluate patient progress.
  • Employ strategies to promote successful return to home, community, and school/vocational environments.
  • Licensed Physical Therapist in the state of Tennessee.
  • Experience in pediatrics, especially with medically fragile infants and children.
  • Acute care or home health experience preferred.
  • Proficient in the use of Electronic Medical Records (EMR) systems for documentation and care coordination.
  • Strong communication and organizational skills.
  • Passion for working with children and families in a home-based setting.
  • CPR certification and valid driver's license required.
