About The Position

Miami Jewish Health is one of the largest providers of healthcare and living options for aging adults in the Southeast. We operate Florida PACE (Program of All-Inclusive Care for the Elderly) Centers, the largest PACE program in Florida, which serves as health plan, healthcare provider, and social center designed for older adults with complex medical needs. Job Summary The Physical Therapist evaluates and reassesses participants to assess mobility level and other functional abilities. Provides and recommends therapeutic exercise, gait training, ambulation, etc. Plans and develops group exercise programs for maintenance therapy for participants. Responsible for the overall Physical Therapy program for Florida PACE Centers.

Requirements

  • Graduate from an accredited school of Physical Therapy approved by the American Physical Therapy Association, the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association or other equivalent organizations approved by CMS
  • One year experience of physical therapy experience working with frail or elderly population
  • Florida State License as a Physical Therapist
  • Current knowledge of safety issues needed in the treatment of participants, especially regarding body mechanics and body substance precautions to protect self, participants and personnel working with the therapists must be maintained
  • Demonstrated human relations and effective communication skills
  • Bilingual in English/Spanish required, Creole a plus
  • Ability to lift 50 pounds
  • Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedure
  • Ensures that resident's/patient's rights are adhered to
  • Demonstrates professionalism and accountability
  • Demonstrates a caring attitude consistent with Miami Jewish EmpathicareSM toward MJH residents, patients, family members, employees, and other facility guests
  • Demonstrates excellent communication skills
  • Ensures confidentiality and security of patients' medical information
  • Identifies and utilizes appropriate channels of communication
  • Able to speak, read and write English
  • Able to think and act calmly to meet unusual occurrences of the job

  • Participates in departmental activities, meetings and in-services and follows established guidelines

Responsibilities

  • Evaluates, identifies, and provides therapeutic interventions as indicated in the care plan
  • Develops an appropriate and comprehensive treatment plan for each participant in order to maximize the participant's potential
  • Reviews participant's status at regular intervals and changes are made to treatment program as necessary
  • Completes assessments and discharge summaries
  • Plans and develops maintenance, exercise, and therapy programs for participants
  • Provides supervision to program aides and other staff who are implementing a Physical Therapy treatment program
  • Participates in and provides in-service education to facilities, supportive staff, students, and the community
  • Documents all treatments rendered and maintain appropriate work records
  • Actively participates as a member of the Multidisciplinary Team, and participates in case conferences
