About The Position

Empath Health is seeking a compassionate Full-Time Physical Therapist to join our interdisciplinary PACE team in Pinellas Park, FL. In this role, you'll help older adults age in place with dignity, delivering personalized therapy across homes, facilities, and our Day Center. This is more than a job—it's a mission. The Program of All-Inclusive Care for the Elderly (PACE) is a unique care model designed to help older adults remain independent and safe in their own homes. Team-Driven, Preventive Focus: Collaborate with physicians, nurses, social workers, and other clinicians in a true interdisciplinary care model focused on prevention and holistic well-being.

Requirements

• Bachelor's degree in Physical Therapy (Master's or Doctorate preferred)
• Active (or eligible) Florida PT license
• Minimum of 1 year of experience working with geriatric populations
• Current CPR certification
• Strong communication, time management, and documentation skills

Responsibilities

• Deliver skilled PT interventions across home, Day Center, and facility setting
• Collaborate with the interdisciplinary care team to create personalized therapy plans
• Track participant progress and manage documentation
• Coordinate use of durable medical equipment (DME)
• Support restorative, maintenance, and rehab goals in a proactive care model
