We are currently seeking a Physical Therapist for our Tully Health Center and Harbor Point locations in Stamford. Requirements

  • Graduate of an accredited curriculum with a minimum of a Doctorate of Physical Therapy.
  • Must be licensed by the Connecticut Department of Public Health.
  • BLS certification required.
  • New graduate may be considered.

Responsibilities

  • Obtains pertinent medical and social history and performs thorough evaluations to measure physical and functional status.
  • Determines functional and measurable short and long term goals and designs an appropriate treatment program.
  • Performs treatment as outlined in the initial evaluation and modifies treatment as necessary.
  • Updates goals and re-evaluates discharge plans throughout the course of treatment.
  • Demonstrates thorough knowledge of treatment techniques and applies them appropriately.
  • Ensures clinical documentation is timely, objective, and complete.
  • Interacts with interdisciplinary staff regarding patient welfare and management.
  • Communicates with patients and families in a respectful and compassionate manner.
  • Assumes personal responsibility for continuing education and professional development.
  • Supervises patient care activities of assistants, aides, and volunteers.
