SUMMARY The Physical Therapist (PT) Inpatient-PRN is responsible for the provision of Physical Therapy services for an inpatient and/ or outpatient population from age 15 and older including, but not limited to, evaluation, evidence-based intervention, equipment recommendations and modality application, under the referral of a physician or qualified non-physician practitioner.

Requirements

  • Must possess a Physical Therapy degree from an accredited program.
  • Physical Therapist IN state license required.
  • Must have current CPR certification in accordance with AHA standards or be able to obtain within 4 weeks of start date.

Nice To Haves

  • 1-year experience with adult physical rehabilitation preferred

Responsibilities

  • Administers PT evaluations to inpatients and outpatients and determines appropriate treatment programs and/or equipment needs, integrating age specific needs and issues for physical medicine rehabilitation population.
  • Develops a patient-centered treatment plan based upon the evaluation, patient goals and age-related issues/needs to optimize treatment and function.
  • Responsible for assuring the treatment plan supports medical necessity and incorporates evidence-based treatments and outcome measures.
  • Accurately completes documentation in compliance with department and regulatory timelines to ensure a complete medical record that supports skilled services and appropriate medical necessity.
  • Other duties as requested.
