About The Position

The Physical Therapist-PACE position provides high-quality services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of patients suffering from injuries or disease as they restore, maintain, and promote overall fitness and health. Responsible for evaluating individuals with mobility dysfunction and providing a plan of care that is specific to the individual's goals related to functional independence. This is a specialized Physical Therapist (PT) position to support the Rehabilitation Manager directly over five major domains of work: staff coverage, staff training, administration, project coordination, and regulatory compliance. The PT will cover various PACE Physical Therapist duties remotely or in-person for any of the AltaMed PACE Centers, as needed to support compliance-related needs such as assessments, care planning, and IDT/Care Coordination meetings. The PT will support training such as onboarding, process implementation, and competency re-training as assigned by the Rehabilitation Manager. The PT will support Rehab operations by assisting processes such as data collection, data analysis, report building, preparing and facilitating presentations, and compliance recordkeeping. The PT will support PACE by assisting with program planning and implementation, participating in CQI initiatives, and serving on interdisciplinary committees to improve participant health outcomes and the delivery of Rehab services. The PT will collaborate with discipline managers, QI team, the Rehabilitation department, and other PACE leaders to align with organizational goals, provide an exceptional customer experience, and optimize care delivery.

Requirements

  • Physical Therapist license issued by the State of California Physical Therapy Board.
  • Minimum of 1 year of experience working with the frail elderly and disabled populations.
  • Current BLS certification.
  • Bilingual English/Spanish/Cantonese/Mandarin preferred.

Responsibilities

  • Evaluate individuals with mobility dysfunction and provide a plan of care specific to individual goals.
  • Support staff coverage and training for PACE Physical Therapist duties.
  • Assist with compliance-related needs such as assessments and care planning.
  • Participate in IDT/Care Coordination meetings.

  • Assist with program planning and implementation for PACE.
  • Participate in Continuous Quality Improvement (CQI) initiatives.
  • Collaborate with discipline managers and other PACE leaders.
