About The Position

The Physical Therapist - Home Health position involves providing care to patients in their homes, focusing on rehabilitation and recovery. The contractor will be responsible for managing a caseload of patients, ensuring they receive the necessary therapy to improve their physical health. The role requires travel within the designated territory, specifically the southern half of Plymouth, MA, and involves a commitment to a 13-week contract assignment. The schedule is Monday to Friday from 8 AM to 5 PM, with at least one weekend day per month required depending on patient volume. The contractor will not be on call and will use the Homecare Homebase (HCHB) EMR system for documentation and patient management.

Requirements

• Licensed Physical Therapist in the state of Massachusetts.
• Experience in home health care.

Responsibilities

• Provide physical therapy services to patients in their homes.
