About The Position

Join the Home Health team as a Physical Therapist, where you'll be accountable for rendering physical therapy services in the comforts of the patient's home, gauging the patient's functional capacities, and formulating remedial programs to enhance self-reliance and physical and cognitive abilities. Key elements of your role include enlightening both patients and caregivers, oversight responsibilities, and routinely documenting patient's advancement.

Requirements

An advanced degree at Master's level or higher
A valid Physical Therapist license in Illinois (IL)
An unblemished Regular Driver License (DL)
A year or more experience in a Healthcare environment

Responsibilities

Conceive and implement physical therapy assessments and treatment protocols
Log and communicate the patient's response to therapy, including changes in condition
Educate patients and their caregivers in the utilization of therapy programs and devices
Oversee the work of physical therapy assistants and certified nursing assistants when warranted
Compile reports and actively participate in staff development and Agency activities
