Requirements

• Licensed Physical Therapist (PT) in the state of New Mexico
• At least 1 year of clinical experience (home health preferred)
• Skilled in patient communication, documentation, and problem-solving
• Self-motivated and comfortable working independently
• Proficient with electronic health records and mobile charting
• Passionate about function-focused, relationship-driven care

Responsibilities

• Evaluate patients and create personalized therapy plans in the home setting
• Deliver evidence-based interventions to restore mobility and function
• Educate patients and families on exercises and safety strategies
• Collaborate with the interdisciplinary team to ensure coordinated, goal-oriented care
• Maintain accurate and timely documentation to meet quality and compliance standards
• Adapt care plans based on progress and physician feedback
• Support patients through discharge planning and wellness strategies
