Requirements
- Licensed Physical Therapist (PT)
- Strong clinical skills and commitment to patient-centered care
- Reliable transportation
Nice To Haves
- Home health/geriatric experience preferred (not required)
Responsibilities
- Perform skilled PT evaluations and treatments in patient homes
- Develop individualized care plans based on patient goals
- Educate patients/caregivers on safety, mobility, and home exercise programs
- Complete timely, accurate documentation (including OASIS when applicable)
- Collaborate with the interdisciplinary team for care coordination
- Maintain professional, compassionate communication and clinical quality
