About The Position

A Home Health Physical Therapist provides skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, balance, and functional independence. This role includes direct patient care as well as completion of federally required OASIS assessments to support clinical outcomes, care planning, and regulatory compliance. The objective of this role is to enhance patient independence and quality of life by delivering skilled physical therapy in the home setting while ensuring accurate OASIS documentation that supports high-quality, compliant, and outcome-driven care.

Requirements

  • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
  • Current state Physical Therapist licensure
  • CPR certification (or ability to obtain)
  • Valid driver’s license and reliable transportation
  • Strong clinical reasoning and independent decision-making
  • Proficiency or willingness to learn OASIS documentation standards
  • Excellent communication and patient education skills
  • Strong organizational and time management abilities
  • Ability to adapt care to varied home environments
  • Compassionate, patient-centered approach to rehabilitation

Nice To Haves

  • Experience with home health care and OASIS documentation strongly preferred

Responsibilities

  • Conduct comprehensive in-home physical therapy evaluations and functional assessments
  • Complete and maintain accurate OASIS (Outcome and Assessment Information Set) documentation at required admission, reassessment, recertification, and discharge points
  • Develop individualized treatment plans based on patient condition, functional limitations, and home environment
  • Provide skilled interventions including therapeutic exercise, gait training, balance training, and functional mobility training
  • Assess home safety risks and recommend adaptive equipment or environmental modifications
  • Educate patients and caregivers on safe mobility techniques, fall prevention, and home exercise programs
  • Monitor patient progress and adjust care plans based on clinical outcomes
  • Collaborate with physicians, nurses, and interdisciplinary care teams to coordinate patient care
  • Ensure timely, accurate documentation in compliance with Medicare and agency standards
  • Travel to patient homes within assigned service areas
  • Functional mobility and ADL assessment
  • Fall risk and safety evaluation
  • Pain and cognitive status documentation
  • Identification of clinical changes impacting plan of care
  • Supporting reimbursement and quality reporting requirements
