Requirements

  • Bachelor's or higher in Physical Therapy from an APTA-accredited program
  • Current, unrestricted California Physical Therapist PT license
  • 2+ years professional PT experience
  • Current CPR certification
  • Valid driver's license and reliable smartphone for EMR use

  • Strong assessment, documentation (EMR), and communication skills
  • Able to work independently in the home setting

Responsibilities

  • Provide in-home physical therapy services to adult and geriatric patients under physician orders
  • Perform comprehensive assessments and functional evaluations; develop and update individualized plans of care
  • Coordinate with an interdisciplinary team to align therapy goals with the total plan of treatment
  • Initiate PT programs and teach family/caregivers specific techniques for safe carryover at home
  • Document timely, accurate visit notes in the EMR; communicate changes in condition promptly
  • Prepare clinical/progress summaries; collaborate on safe discharge planning and continuity of care
  • Adhere to agency, state, and federal regulations and professional standards of practice
