Physical Therapist – Adult Home Care | Per diem Location: Serving Chittenden & Grand Isle Counties, VT ️ Make a Difference at Home Are you a passionate Physical Therapist looking for a rewarding career in home health? Join UVM Health’s Adult Home Care team and help patients maintain independence, improve mobility, and enhance overall well-being—all in the comfort of their own homes . We are looking for therapists that are available weekdays. As a home health physical therapist, you’ll be part of a collaborative interdisciplinary team including RNs, LPNs, LNAs, Occupational Therapists, Speech-Language Pathologists, Social Workers, and administrative staff 🤝, providing comprehensive, patient-centered care. About the Role As an Adult Home Care Physical Therapist, you will: Our coverage area is segmented into four territories within Chittenden and Grand Isle Counties. Case load territory is assigned after orientation. The University of Vermont Health Network/Porter Medical Center is located in the heart of beautiful Middlebury, Vermont. We are comprised of a 25-bed acute care facility, a 98-bed skilled nursing facility which includes a memory care unit, short-term rehabilitation unit and a long-term care unit, and a network of primary care and specialty medical practices throughout Addison county and the town of Brandon.

Requirements

  • Bachelor’s degree or higher in Physical Therapy
  • 2+ years of general PT experience preferred, or 1 year at PT I level within UVM Health Home Health & Hospice with strong performance
  • Current Vermont Physical Therapist license, or willingness to obtain one

Nice To Haves

  • Home health experience preferred, but not required, we can train the right person!
