As a key member of our Home Health Rehab team you will further our mission by: Serving as a liaison between Community Medical Centers and the community by initiating, intervening, and evaluating physical therapy services to patients in their home. Requirements

• Bachelor's Degree in Physical Therapy
• State of California Physical Therapist License or eligible for licensure
• Valid California Driver's License and driving record acceptable to Community Medical Centers required
• Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA)

Nice To Haves

• 1 year of experience in an inpatient or outpatient clinic setting

Responsibilities

• Evaluating, planning, organizing, and implementing specific treatment programs for individual patients.
• Implements therapy according to established plan, utilizing appropriate procedures, equipment and techniques within the scope of physical therapy.
• Educates the patient and/or the caregiver about the findings of the evaluation, established goals of the physical therapy program, plan of treatment, home programs, injury prevention, and other related information.
• Communicates effectively with other patient care team members and coordinates care provided.
• Supervises PT Assistants with implementation of treatment plan.
