About The Position

As a Physical Therapist Home Health PRN with HCA Florida Healthcare at Home, you will be part of an organization devoted to giving back to the community. The role involves providing high-quality, patient-centered care by managing the complete process of therapeutic care for an assigned group of patients in coordination with the physician and the patient care team. You will complete patient assessments and participate in developing and implementing individualized care plans focusing on goal achievement, patient engagement, and positive patient experience.

Requirements

• Bachelor of Science in Physical Therapy from an accredited program.
• Completion of the National Certification Examination for Registered Physical Therapist.
• License to practice physical therapy within the state.
• Ability to work varying hours, including weekends and holidays.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.

Responsibilities

• Provide evaluations and treatment interventions within the scope of practice established by law and professional code of ethics.
• Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
• Provide purposeful and skilled therapeutic care using procedures and techniques to produce a change in condition/function consistent with the physician's plan of care.
• Instruct patient/family/caregiver in the use of therapies, exercises, therapeutic appliances, and/or prosthetic devices to be used outside of therapy sessions.
• Prioritize caseload, determine daily staffing needs and coordinate assignments/handoffs.
