About The Position

The position of Physical Therapist (PT) in Home Health Services is a per diem role located in Greenville. The job involves providing evaluative, consultative, and treatment interventions to home health patients referred by a licensed physician, while maintaining effective communication with patients, their families, and other health partners.

Requirements

  • Must have a valid professional license in State of practice.
  • Graduate of an accredited Physical Therapy Program.
  • If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.S. by a credentials evaluation organization approved by the APTA.

Nice To Haves

  • Prior Home Health experience is preferable.

Responsibilities

  • Evaluate the patient's level of function and skilled therapy needs.
  • Treat the patient to relieve pain, develop or restore function, and obtain maximum outcomes according to the established plan of care.
  • Advise and consult the family and other interdisciplinary team members to promote optimal functional outcomes.
  • Assess, monitor, document, and report progress of patient's health and condition using required documents via clinician preferred technology, and within established timelines.
  • Help maintain continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan.
  • Demonstrate intermediate knowledge of home care and competency in discipline for specific patient care skills.
  • Conduct field visits to evaluate & initiate start of care and conduct clinical outcomes monitoring.
  • Utilize accepted company teaching materials in patient/family education.
  • Complete documentation timely, accurately, and at the point of care, according to industry standards.
  • Meet agency productivity standards and utilize time and resources effectively.
  • Participate in center/agency surveys, mandatory in-services, case conferences, and staff meetings.
  • Follow established safety regulations and procedures, and report job-related functions/tasks that involve occupational hazards.
  • Maintain current license, certification, and clinical records to meet federal, state and agency regulations.
  • Recognize and respond to priorities in patient care showing initiative and sound judgment.
  • Demonstrate willingness to adapt to change in workload and assist with training/orienting others.
  • Accept responsibility to report any alleged or suspected violations to supervisor and/or others.
