About The Position

As a Home Health Physical Therapist at CenterWell Home Health, you will play a crucial role in providing skilled physical therapy treatment to patients with various injuries, illnesses, and functional disabilities. Your primary goal will be to help patients achieve the highest level of physical function through personalized rehabilitation programs and effective communication with the healthcare team.

Requirements

• Degree from an accredited Physical Therapy Program (approved by the APTA).
• Minimum of one year physical therapy experience preferred.
• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Strong organizational and communication skills.
• A valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Plan and administer prescribed skilled physical therapy treatment and training for patients.
• Test/screen the patient's physical strengths to assist in evaluating their level of function.
• Develop and implement a conditioning/rehabilitation program consistent with the physician's Plan of Treatment.
• Confer with physicians and clinical team members to gather patient information and revise treatment programs as necessary.
• Provide written instructions and care plans to Physical Therapy Assistants and Home Health Aide staff.
• Supervise and evaluate the performance of Physical Therapy Assistants and Home Health Aides.
• Accurately document patients' care observations, interventions, and evaluations.
• Report patient's progress to the physician, Clinical Manager, staff, patient, and family.
