About The Position

The Home Health Physical Therapist provides direct patient care in the patient's home environment as part of a multidisciplinary team including nursing, physical therapy, occupational therapy, speech, social work and home health techs. The Physical Therapist evaluates patients and uses professional judgment to design the individualized plan of care which best meets the patient's needs. The Physical Therapist provides direct, one-on-one patient care, and will also supervise care provided by assistants. The Home Health agency relies heavily on technology; email is a primary means of communication, and the therapist will use a laptop to complete documentation in an electronic record. The patient population varies widely across the spectrums of acuity, age, and diagnosis. The Physical Therapist works under the direction of a clinical team manager with support from a rehab clinical coordinator. Employment begins with a 4–6-week orientation with a seasoned preceptor to ensure that the clinician is well prepared when they begin treating on their own. Additional job duties include performance improvement opportunities, policy and procedure review and development, research, etc. Working in home health provides endless variety of setting, scenery, and clinical challenges, and provides a unique opportunity to enter and work within the real-life environments of our patients, bringing meaningful change to their daily lives.

Requirements

  • Bachelor's, master's or doctoral degree in Physical Therapy.
  • Completed CI credentialing course or equivalent.
  • Minimum two years of clinical experience (4,160 hours).
  • Physical Therapy licensed or eligible in the state of assignment (Virginia or West Virginia).
  • BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved.
  • Valid driver's license and reliable transportation.
  • Basic computer and keyboarding skills.
