About The Position

Mass General Brigham Home Care provides post-acute, medically necessary skilled care in the home to help homebound patients recover from illness, surgery or injury and manage exacerbations of chronic disease. Services include skilled nursing care, physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies. We are seeking a full-time, 36-hour Physical Therapist to join the “Ironworks” team. This position will work 4 - 9-hour days, hours are 8am - 5:30pm. This position will work in Lynn, Nahant, and Saugus as well as supporting neighboring towns as necessary. While navigating transportation and parking in the city can be challenging, we will secure city parking passes for new team members to make the commute as convenient and stress-free as possible! Enjoy the freedom to design your own schedule while delivering 1:1 care - because your work-life balance matters. Our Physical Therapists create an initial care plan and do not case manage. They collaborate closely with PT case managers, nurses, and social workers. You’ll typically complete 3 Start of Care visits per day, with time for documentation, chart review, and coordination built into your schedule. Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease or other causes. Our physical therapists will see adult and geriatric patients. Collaborate with patient, family, physician and other allied health professionals to identify patient needs and coordinate services to meet those needs. Requirements

  • One year of experience in acute care, rehab or homecare as a Physical Therapist
  • Registered with the Board of Registration in Massachusetts
  • Excellent verbal, written and electronic communications skills
  • Computer proficiency and ability to learn new software
  • Ability to prioritize work and adapt to changing situations
  • Travel required within Mass General Brigham Healthcare at Home’s geographic area

Responsibilities

  • Develop a plan of care that is patient and family focused, and institutes standardized interventions for goal-oriented outcomes
