Location: This position serves home care patients in Minnetonka, Hopkins, Eden Prairie, Chanhassen, Mound, and Victoria, MN. Pay rate: Up to $75/hour Depending on Experience! This is a temporary assignment expected to start ASAP and will last at least through June 2026 with the potential of it extending longer.

Requirements

  • Must have a current license with the Minnesota Board of Physical Therapy.
  • Must have a Bachelors, Masters or Doctorate degree in Physical Therapy.
  • At least one year of previous experience as a PT in home care settings.
  • Must be comfortable/familiar with Medicare visits and have knowledge of OASIS Start of Care process,
  • Must demonstrate knowledge of age related growth and development principles to provide appropriate service regarding primary diagnosis and multiple co-morbidities affecting care.
  • Proficiency with communication and data processing equipment, clinical apparatus related to home care, electronic medical record system, OASIS evaluation and data set.
  • Must have reliable transportation for client visits.

Responsibilities

  • Assess physical, psychological, social and environmental needs of the patient and family.
  • Integrate principles of development and disease process including co-morbidities in creating the plan of care.
  • Maintain necessary records and use them appropriately for the care of patients, and demonstrate proficiency in the OASIS data set time points.
  • Provide patient/family education including patients self care and care provided by family.
  • Serve as a case manager for assigned patients and coordinate with all providers involved in patient care including RN, SW, home health aide and other rehab disciplines.
  • Assist patients and families in resolving concerns or complaints of care or service, and provide direction and supervision of care by home health aides.
  • Assess needs and refer to Adult Protection Services regarding status of vulnerable adults as necessary.
  • Provide skilled clinical care including evaluation and treatment of patients, and the development/implementation of care plans.
  • Anticipate the needs of the population served as well as the unique characteristics of the communities served.
  • All other duties as assigned.
