About The Position

A Home Health Physical Therapist provides skilled rehabilitation services to patients in their homes. The role focuses on restoring movement, improving functional ability, reducing pain, and helping patients safely remain independent in their home environment. The objective is to improve patients’ mobility, safety, and independence by delivering high-quality physical therapy services in the home setting while supporting overall recovery and long-term function.

Requirements

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
• Current state licensure as a Physical Therapist
• Valid driver’s license and reliable transportation
• CPR certification required or ability to obtain
• Strong clinical reasoning and independent decision-making skills
• Excellent communication and patient education abilities
• Ability to adapt treatment approaches to varied home environments
• Strong time management and organizational skills
• Compassionate and patient-focused approach to care

Nice To Haves

• Home health or outpatient experience preferred

Responsibilities

• Conduct in-home physical therapy evaluations and develop individualized care plans
• Treat patients recovering from surgery, injury, illness, or managing chronic conditions
• Implement therapeutic exercises, mobility training, balance and gait training, and functional strengthening
• Assess home safety and recommend modifications or assistive devices as needed
• Educate patients and caregivers on exercises, safety techniques, and injury prevention strategies
• Monitor patient progress and adjust treatment plans based on outcomes
• Maintain accurate and timely documentation in accordance with regulatory and agency standards
• Coordinate care with physicians, nurses, and other rehabilitation professionals
• Travel to patient homes within assigned territory
