Our Home Health Care team provides compassionate, person-centered rehabilitation services directly in the homes of the individuals we serve. Position Summary We are seeking a dedicated and compassionate Physical Therapist to join our Home Health Care team. In this role, you will deliver one-on-one therapeutic services to older adults in their homes, helping them regain mobility, improve function, and maintain independence. This position offers autonomy, flexibility, and the opportunity to make a profound difference in the lives of seniors and their families. As a nonprofit organization, Mozaic Senior Life is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program .

Requirements

  • Current Connecticut Physical Therapist (PT) license in good standing.
  • Doctor of Physical Therapy (DPT) or Bachelor's/Master's in Physical Therapy.
  • Strong communication, critical-thinking, and patient engagement skills.
  • Ability to work independently and manage a flexible schedule.
  • Valid driver's license, reliable transportation, and ability to travel within service areas.

Nice To Haves

  • Previous experience in home health, geriatrics, rehab, or related settings preferred.

Responsibilities

  • Conduct comprehensive evaluations to determine patients' functional needs and develop individualized treatment plans.
  • Provide evidence-based physical therapy interventions in the home setting.
  • Educate patients and caregivers on exercises, safety techniques, and equipment use.
  • Collaborate with an interdisciplinary team including nurses, OT, SLP, social workers, and physicians.
  • Document assessments, progress notes, and care plans in accordance with regulatory and organizational standards.
  • Adjust treatment plans as needed to ensure optimal outcomes and patient satisfaction.
  • Participate in case conferences, team meetings, and quality improvement initiatives.
  • Ensure compliance with all home health care regulations and agency policies.
