About The Position

This position is responsible for providing Physical Therapy services to Tabitha's Home Health Care Seniors/residents/clients (hereinafter, “Seniors/clients”) with physical or cognitive disabilities to maximize functional independence. Requirements

  • Must be able to speak, read, write, and understand the English language including medical terminology related to the job.
  • Bachelor's Degree in Physical Therapy required.
  • Physical Therapist license in good standing as recorded on the Nebraska State Registry required.
  • CPR Required.
  • Valid Nebraska Driver's license and a good driving record required.
  • Must possess auto insurance at or above Nebraska statutory requirements.
  • One year experience as a Physical Therapist, preferred.
  • Geriatric experience, preferred.

Responsibilities

  • Evaluates clients utilizing appropriate diagnostic muscle, nerve, joint and functional ability tests.
  • Performs Physical Therapy treatment procedures or delegates to a licensed Physical Therapist Assistant based on initial and on-going client evaluation.
  • Develops and implements the client's plan of care in collaboration with the physician, other consulting health care professionals, family/caregiver according to agency procedures and state and federal regulations.
  • Documents the care provided, instructions given, client's progress toward goals, and changes in condition adhering to Tabitha's policies and procedures and state/federal regulations.
  • Reports significant changes in the client's condition to the physician and nursing staff.
  • Communicates and coordinates care and client's progress with other team members involved in the care.
  • Designs, implements, and evaluates therapeutic exercise programs, and instructs the client's family/caregiver/Home Health Aide in carrying out the program.
  • Consults with the attending physician and obtains a written or telephone order for Physical Therapy plan of care and any changes in the client's plan of care as needed.
  • Ensures that assigned client's clinical records are complete, timely, and reflect coordination of care.
  • Oversees and coordinates Physical Therapist Assistant and Rehabilitation Aide client care activities.
  • Organizes and prioritizes work to complete responsibilities according to plan within established time frames to meet the client's medical and social needs.
  • Communicates effectively with clients, families, staff and support services to achieve client goals and meet service expectations.
  • Maintains work schedule which allows for coverage in all Rehabilitation programs as needed.
  • Attends and actively participates in client care conferences as indicated whenever on duty.
  • Plans, coordinates, supervises, and evaluates all student interns and volunteers.
  • Teaches in-services and orientation of nursing employees relating to Rehabilitation upon request of supervisor.
  • Willingly takes on additional duties and/or assignments, assisting with projects and responding to requests as assigned or as necessary.
