Requirements

• Current Physical Therapy licensure in state of practice.
• 2 years of appropriate experience as a physical therapist.
• Satisfactory grade on a proficiency examination.
• Auto liability insurance.
• Clean driving record.
• Current driver's license.
• Must register with and successfully pass the Family Care Safety Registry upon hire.
• Must be able to successfully pass KDADS criminal record check upon hire.
• AHA BLS for Healthcare Providers within 90 days.

Responsibilities

• Independently evaluates patients and develops a treatment plan based on goal directed objectives.
• Delivers and documents care, and provides discharge planning on all physician referred patients.
• Supervises and directs the treatment and activity of support staff.
• Performs tasks in compliance with the professional code of ethics, standards of practice, state/national regulations, hospital standards, and departmental policies/procedures.
• Coordinates the workflow through the Home Health therapy department during the absence of the Lead PT, by planning daily staffing and schedules of assigned coverage.
