Responsibilities: Provides skilled physical therapy in the home setting to patients ranging in age from adolescent through geriatric. Responsible for the overall management and of an assigned case load of patients, coordinating care with physical therapy assistants, multiple disciplines, physicians and community resources to achieve highest level of physical independence. Conducts physical therapy evaluations, modalities and treatments to restore function, relieve pain and prevent disability following illness or injury. Practices in compliance with professional knowledge and standards of clinical practice and in accordance with policies and procedures of the agency, laws and regulations of Medicare, Medicaid and other third party payers. Demonstrates professional growth and accountability. Reports to and is supported by the Piedmont Home Health Clinical Manager. Qualifications: Education Graduate of a Physical Therapy program accredited by The Commission on Accreditation in Physical Therapy (CAPTE) or predecessor/successor organizations with a BS, MS, or DPT Required
