About The Position

FlexStaff is seeking a dedicated Physical Therapist to join our client’s dynamic team and deliver exceptional therapy services across our community. This is a full-time temporary position located in Ridgewood, NY, aimed at helping older adults regain mobility, reduce pain, and prevent disability through compassionate, expert care.

Requirements

  • A licensed Registered Physical Therapist in New York State with a passion for working with the elderly.
  • At least 1 year of experience, preferably with a geriatric or frail population (training provided if less experienced).
  • Skilled in building rapport with patients and working collaboratively within a team.
  • Able to travel to participants’ homes, nursing homes, and clinics as needed.
  • Knowledgeable about PACE regulations and familiar with EMRs like Care Compass (a plus!).
  • Bilingual skills are a plus, depending on participant demographics.
  • Successful completion of an accredited Physical Therapy program.
  • NY State licensure as a Registered Physical Therapist.
  • Ability to promote a positive, trusting relationship with patients and staff.
  • Medically cleared for communicable diseases and immunizations up-to-date.

Nice To Haves

  • Experience with geriatric or frail populations.
  • Familiarity with EMRs like Care Compass.
  • Bilingual skills depending on participant demographics.

Responsibilities

  • Assess and develop personalized treatment plans for patients referred by healthcare providers.
  • Perform evaluations and deliver hands-on therapy at our PACE site or in the field—at participants’ homes, nursing facilities, and more.
  • Collaborate with an interdisciplinary team to create comprehensive care plans that promote independence and well-being.
  • Document all assessments, progress notes, discharge summaries, and care plans accurately and in accordance with departmental policies.
  • Participate in discharge planning, including recommending equipment to support ongoing mobility and safety.
  • Lead clinical instruction for Level I & II Physical Therapy students, Assistants, and volunteers, sharing your expertise and fostering future professionals.
  • Contribute to staff training and performance improvement initiatives to ensure the highest standards of care.
  • Maintain a safe, clean environment following all safety protocols.
