About The Position

How You’ll Help Transform Healthcare: Confirms and interprets physician referrals, performs patient assessments and plans and implements individualized treatment programs taking into account the patient’s developmental and/or chronological age and medical diagnoses in accordance with the regulations governing the principles and practice of Physical Therapy. Provides patient care based upon physician referral and age-appropriate assessment taking into consideration the patient’s developmental/chronological age and medical diagnoses when planning/implementing treatment programs designed to achieve goals set in conjunction with the patient and/or family member. Coordinates patient care and clinical activities through written and/or verbal communication with patients, families, therapists, Clinical Team Leaders, managers, physicians and other members of the health care team. Documents in the patient’s medical record, and completes other patient care-related forms in compliance with regulatory agencies and department/hospital policies and procedures. Supervises PTAs and collaborates with other therapy staff, support staff, volunteers and students to achieve efficient clinical activity and quality patient care. Participates in other patient care and departmental/professional activities as requested/assigned including but not limited to presenting/attending in-services, performance improvement, program development/evaluation, new employee orientation, committee membership, peer review and attending staff meetings. Supervision – Supervises Physical Therapist Assistants (PTAs) as provided by State of Virginia licensure/practice standards and Federal regulations. Orients new employees and may provide guidance to volunteers, students, support, or contract staff. Communications – Communication commonly occurs with persons who have similar or different functions inside or outside the organization. May provide problem-solving, consultation, advising or interpretation services to others. May work with outside vendors, but does not have the authority to commit for the organization. Confidentiality – Access and exposure to patient information is required for the performance of the job. Financial/ Budgetary – Suggests cost containment initiatives in immediate work unit/department. Judgment – Within scope of license, age-specific criteria and per physician referral, interprets physician orders, performs patient assessments, and plans and implements individualized patient treatment programs. Position impact – Impact of work is generally within department or service line. Working Conditions – Work is performed primarily indoors in a clinical setting including hospital, nursing home or outpatient sites. May be required to work at other Carilion facilities to meet staffing needs, in which case will provide own transportation. For Home Care Departments work is performed at patient’s home. On occasion work may be performed outside of a clinic setting such as work with the driving program, hydrotherapy or home evaluations. Work requires physical activity with patients including lifting, manipulating, pushing, pulling, bending and walking. Patients may weigh over 200 pounds. Occasional exposure to serious injury and frequent environmental hazards to include blood and bodily fluids requiring standard precautions. Home care practioners must provide own transportation and travel daily between patient homes. Age-specific criteria - Demonstrates knowledge of the principles of life span growth and development and assesses data reflective of the patient’s status. Interprets the appropriate information needed to identify each patient’s requirements relative to his or her age specific needs and provides care according to departmental policies and procedures Other - May be required to work at other Carilion facilities to meet staffing needs.

Requirements

  • Education: Graduate of approved school of Physical Therapy. Bachelor's degree as Physical Therapist and 5 years of experience, OR Master's or Doctorate degree required.
  • Experience: Experience in site specific level of care preferred but not required. For Home Health positions one year of clinical experience required.
  • Licensure, certification, and/or registration: Licensed to practice in the Commonwealth of Virginia by the Virginia Board of Physical Therapy.
  • Life Support: AHA BLS-certified (from a Carilion approved program) within six months of employment.
  • Other Minimum Qualifications: Effective interpersonal, communication, customer service and teamwork skills required.
