About The Position

The Physical Therapist Evaluator position is available in various school districts throughout Eastern Suffolk, NY. This role is fee-for-service, with flexible scheduling from Monday to Friday, following the school calendar. Compensation is set at $160 per evaluation, with rates determined by the service provided, professional experience, and licenses/certifications held. The position may be either full-time or fee-for-service, depending on the current caseload, which will be discussed during the interview. This arrangement can vary and is subject to change.

Requirements

• Graduate from an accredited university with a degree in Physical Therapy.
• Current New York State Physical Therapy License (DPT).
• 3+ years experience providing PT services and evaluations to school-age students (5-21).
• Familiar with standardized assessments such as the BOT-3, DTVP-3, Beery VMI, and Sensory Profile.

Nice To Haves

• Bilingual is a plus.

Responsibilities

• Administer and write up occupational therapy evaluations on agency provided template forms and submit in a timely manner.
• Update student IEP drafts and create appropriate goals, as needed.
• Ongoing communication with lead supervisors & coordinators, specifically related to case management.
• Maintain professionalism with families, school teams, and other professionals.
• Maintain accurate and timely attendance and billing logs.
• Create and maintain schedules with assigned individual families/school personnel.
• Provide input and attend/teleconference initial CSE meetings and/or annual reviews.
