The position involves facilitating the restoration of function and prevention of disability for patients with rehabilitation needs and/or physical disabilities by planning and administering medically prescribed physical therapy. The role requires a comprehensive approach to patient care, including assessment, planning, implementation, and evaluation of rehabilitative care, as well as providing information and support to patients and healthcare teams.

  • Identifies patient rehabilitation care requirements by assessing patient condition upon order of physician and at regular intervals during assigned shift.
  • Plans, implements, and evaluates rehabilitative care by setting goals, collaborating with healthcare team, patients, and families, acting on physician's orders, documenting interventions, and revising care plans based on patient response.
  • Provides information to patients and healthcare team by meeting them, providing educational information related to treatments and procedures, and answering questions.
  • Monitors delivery of care by completing patient rounds, documenting care, identifying process improvements, intervening to overcome deviations in the expected plan of care, and maintaining ongoing communication with care management.
  • Assures quality of care by fostering practices that adhere to hospital's philosophy and standards, measuring outcomes against patient care goals, and making necessary adjustments.
  • Protects self, co-workers, and patients by interpreting and implementing policies to prevent the spread of diseases and adhering to infection-control policies.
  • Respects patients by recognizing their rights, providing emotional support, and maintaining confidentiality.
  • Maintains a safe and clean working environment by complying with procedures and keeping patient rooms and therapy areas orderly.
  • Ensures operation of equipment by following manufacturers' instructions, troubleshooting malfunctions, and maintaining equipment inventories.
  • Maintains supplies inventory by checking stock levels, anticipating needed supplies, and placing orders.
  • Maintains professional and technical knowledge by attending workshops, reviewing publications, and participating in professional societies.
  • Contributes to the team effort by accomplishing related results, following through on delegated tasks, and responding to coworkers' requests for assistance.
  • Promotes the development of Good Shepherd as a system by volunteering at community events and acting as a preceptor for new associates.
  • Bachelor's Degree in Physical Therapy required.
  • Previous acute care experience or similar setting required.
  • Physical Therapist license required.
  • Basic Life Support / CPR certification as a healthcare provider per the American Heart Association required.
