About The Position

Desert Terrace Healthcare Center is seeking a full-time Physical Therapist (PT) to join our in-house therapy team in Phoenix, AZ. This position offers a unique opportunity to work in a busy and dynamic skilled nursing facility, where you will be part of an experienced and established team. In this role, you will engage in clinical mentorship and job shadowing, contributing to clinical and program development while also participating in leadership development initiatives. As a Physical Therapist, you will be responsible for effectively screening and evaluating patients with cognitive and/or Activities of Daily Living (ADL) physical functioning disorders. You will develop appropriate plans of care that adhere to all regulatory and clinical practice standards. Utilizing standardized assessments and evidence-based practices, you will provide rehabilitative, skilled, and medically necessary treatment interventions to patients. Your role will also involve administering various procedures as part of the rehabilitation plan, assessing patient progress, and modifying treatments as necessary. You will communicate effectively with supervisors and other health team members regarding patient progress and treatment plans, and provide comprehensive discharge summaries of services rendered.
