About The Position

LOGAN Community Resources is a non-profit organization dedicated to supporting people with intellectual and developmental disabilities in Michiana. The Physical Therapist will provide evaluations and therapy in various settings, focusing on the gross motor development of children. This role involves creating treatment plans that align with each child's Individual Family Service Plan and communicating progress with families and staff.

Requirements

  • Licensed registered Physical Therapist in the State of Indiana.
  • One year of experience working with the pediatric population preferred.
  • Ability to communicate effectively with staff, supervisors, and agency personnel.
  • Ability to establish and maintain effective relationships with children and families.
  • Ability to provide developmental outcomes and demonstrate activities to families.
  • Ability to work independently and use sound judgment.

Responsibilities

  • Assist in evaluating children on an individual basis.
  • Communicate results of assessments and screenings.
  • Determine each child's adaptive and orthopedic equipment needs.
  • Conduct individual therapy to maintain or enhance speech and language abilities.
  • Facilitate parental growth as teachers by providing necessary resources and support.
  • Provide a developmental model of learning to parents.
  • Maintain daily progress notes and quarterly reports.
  • Write discharge summaries for children transitioning to the school system.
  • Participate in case conferences, staff meetings, and parent group meetings.
  • Attend workshops and conferences to meet professional development requirements.
  • Serve as a resource for information on gross/fine motor skills and necessary equipment.
  • Plan instructional activities and community activities.
  • Respect confidentiality and cultural diversity of children and families.
