About The Position

Join Iowa Home Care as a Physical Therapist Assistant, where you'll provide essential therapy services directly in patients' homes. Working closely with a physician-directed plan, you'll assess and improve patients' strength, mobility, and functional status to support recovery and independence. This role emphasizes patient-centered care, collaboration, and adherence to high-quality standards.

Requirements

• Degree in Physical Therapy from an accredited program (Bachelor's or Master's).
• Licensed to practice as a Physical Therapist in the state.
• Minimum of two years of experience in physical therapy; community/home health experience preferred.
• Strong communication, organizational, and problem-solving skills.
• CPR certification and a valid driver's license with reliable transportation.

Responsibilities

• Deliver physical therapy treatments according to physician orders and the state Physical Therapy Practice Act.
• Evaluate patients' therapeutic needs and develop personalized care plans, adjusting as needed.
• Educate patients and caregivers on the use and care of therapeutic equipment.
• Report patient progress and any changes in condition to the supervising physician.
• Participate in discharge planning and submit clinical progress reports.
• Supervise physical therapy assistants and collaborate with other healthcare team members.
