Schedule: Monday - Friday 8:15-5p Territory: Butler County and Hamilton County Agency: The Christ Hospital Home Health Care SUMMARY The Physical Therapist Assistant (PTA) provides comprehensive skilled services under the direction of a licensed Physical Therapist within a designated geographic territory. The PTA educates patients and their families, prioritizing patient safety at all times. This role is responsible for maintaining accurate documentation and ensuring compliance with all applicable legal rules and regulations. The Company follows the physical therapy scope of practice as defined by state guidelines for licensed PTA personnel. Additional benefits include: Mileage reimbursement Additional bonus opportunities Flexible scheduling Full team support (including scheduling and patient admissions) Our comprehensive benefits package includes: Medical, dental, and vision insurance with flexible options Paid time off (for eligible employees) 401(k) participation Company-paid life insurance Employee Assistance Program (EAP) QUALIFICATIONS & ATTRIBUTES Current Physical Therapist Assistant license in the state of employment Minimum of one year of PTA experience in an acute care setting (required) Home health experience (preferred) Strong communication skills with patients, families, physicians, and coworkers Knowledge of therapy best practices Sound clinical judgment and decision-making skills Strong problem-solving abilities Detail-oriented and observant Excellent time management and prioritization skills Valid driver’s license and active auto insurance with the employee listed as a driver MAJOR AREAS OF RESPONSIBILITY Provide skilled therapy services in patients’ homes based on the Physical Therapist’s plan of care Observe, document, and report patient responses, changes, and progress Perform prescribed physical therapy procedures and diagnostic tests Deliver treatments to relieve pain, restore function, and maintain optimal performance using methods such as exercise, massage, heat, and hydrotherapy Assess functional limitations and implement interventions in collaboration with the Physical Therapist Communicate effectively with physicians, therapists, and team members Participate in case conferences to support coordinated patient care Recommend or evaluate prosthetic and orthotic devices; perform minor adjustments or suggest modifications Educate staff, patients, and families on therapy techniques, safety measures, and supportive care activities Instruct patients and caregivers on proper use of assistive devices (e.g., wheelchairs, walkers, braces, crutches, canes) Promote patient independence and reduce reliance on agency services Complete all clinical documentation in accordance with agency protocols and Medicare/federal guidelines Conduct chart audits as requested Communicate schedule changes promptly with the scheduling team Participate in on-call duties, training sessions, and mandatory meetings Assist in onboarding and training new employees ESSENTIAL FUNCTIONS Travel to patient homes as scheduled Maintain consistent attendance at assigned visit locations Complete documentation within required timeframes: Routine, discharge, evaluation, recertification, and resumption visits: within 24 hours Start-of-care visits: within 48 hours Submit all required documentation in accordance with company standards Follow the plan of care within the PTA scope of practice as defined by state regulations Compensation Note: Per-visit compensation includes payment for all activities associated with patient care, including preparation, travel, documentation, communication, and coordination with healthcare providers.
